An unannounced monitoring inspection was conducted from 4:15 p.m.-5:15 p.m. with the site director and three staff. 22 children, ranging in age from 5 to 12 years, were in the care of four staff. Children were observed playing basketball, drawing, playing board games, eating afternoon snack, and playing "Red Light, Green Light." Afternoon snack today was graham crackers and apples. The last fire drill was 10/9/15. Seven children's records were reviewed and in compliance. Two staff records were reviewed. Standard #: 22VAC40-185-160-C Description: Based on review of two staff records, the center did not obtain updated tuberculosis (tb) tests or screenings for each staff every two years as required. Evidence: Staff A's last tb test or screening was dated 8/11/13. Plan of Correction: Staff A will get a tb test.
