Technical Assistance:
Consultation provided on the need to obtain documentation that each staff meets the educational requirements for their positions, such as copies of a high school diploma or equivalency.

Comments:
An unannounced monitoring inspection was conducted today with the director. Forty three children were present with 5 staff. One small group of children were in the Power Hour ( homework ) group while the majority of the children were gathering in the large gym upon their arrival from school. The staff and children then divided into smaller groups ( depending on age) and the groups went to the computer room, dance room and some remained in the gym. Three staff and children's files were reviewed today. Fire inspection: 4/14/15 Health inspection: 4/2/15 Arrival time: 4:10 pm Departure time: 6:40 pm

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation and interviews, it was determined that attendance records are not being maintained on children in each group. Evidence: Once the children arrive they gather in the gym, then subsequently break out into assigned groups. Once the children are in assigned groups the staff does not keep a specific attendance log as to which children are is in each group.

Plan of Correction: We will start to keep specific attendance logs per each group.

Standard #: 22VAC40-185-560-F
Description: Based on an inspection of the menu and an interview with the director, it was determined that the snacks being served does not meet the current age appropriate nutritional requirements. Evidence: Upon reviewing the current week snack menu it was determined that snacks provided to the children are one component and not the two components required. The menu was as follows: 10/26th: special k bars and water, 10/27th: cheeze-its and water, 10/28th: granola bars and water and 10/29th: fruit snack and water. Water is not considered a second component.

Plan of Correction: We will change the menu and make sure we are provided two components for snacks.

Standard #: 22VAC40-185-560-N
Description: Based on observation, it was determined that children are allowed to drink while walking around. Evidence: Two children were observed walking around the gym drinking water, these children were not redirected to discontinue this behavior.

Plan of Correction: We will talk with staff and children about children not being allowed to walk around while drinking.
