22VAC40-191 Background Checks (22VAC40-191)
- Technical Assistance:
The center would like to modify their license to be able to take 5 children under age 2.5. They would like to modify to an age range of 2-12. Discussed the need for the center not to accept children that are not within the age stipulated on their license.
16 children present with 3 staff during this monitoring visit. The children are doing puzzles, making pumpkin hats, spiders and 3-d pumpkins. Circle time with stories and singing and music was observed. The children had lunch during the visit. The center was renovated in July and the whole center has new floors and the downstairs are now a large open space with a classroom and kitchen. Interaction between staff and children observed during the visit. Lunch today was meatballs, rolls, green beans and apple sauce. This visit was started at 10: 45 and ended at 12:00. Standard #: 22VAC40-185-40-E Description: The center failed to follow the terms of the current license. Evidence: The center enrolled a child and allowed the child to start in June of 2015 , the child did not turn 2 until 10/24/15. The center is licensed for ages 2.5 to 5. The center received a letter for the city of Fairfax fire department saying they can care for 5 children under 2.5 years of age but failed to ask the licensing office for a modification before enrolling the child. Plan of Correction: We will ask for a modification to allow for 2 year olds to attend and we will not accept children under two.
Standard #: 22VAC40-185-60-A Description: The center failed to get the required information for some children's records. Evidence: Based on the review of 4 children's records the following was found missing: Child B is missing a second emergency contact Child D is missing proof of ID and two emergency contacts, the emergency contacts listed are the parents of the child. Plan of Correction: We will ask the parents for the information.
Standard #: 22VAC40-185-70-A Description: The center failed to get all required paperwork for a new staff member. Evidence; Based on the review of 3 staff files , one file , staff A is missing the child protective service check, references and the sworn disclosure. Plan of Correction: I will make sure we get all the paperwork needed. We will send the CPS check in.
Standard #: 22VAC40-185-550-D Description: The center failed to conduct shelter in place drills for the last year. Evidence: based on the review of emergency drill records and discussion with the director, the center did not have any shelter in place drills in the last year. Plan of Correction: We will make sure we have them twice a year.
