Comments:
An unannounced monitoring inspection was conducted on 10/8/2015. It began at 8:45am and concluded at 10:45am. There were 15 children in the direct care of 5 staff members. Children were observed being fed, changed, reading books, having circle time, singing, and washing their hands. A sampling of staff records, children's records, the physical plant, evacuation drills, and the playground fencing were inspected and observed. Description: Based on record review, staff records were not complete Evidence: Staff #1 file did not contain emergency contact information or orientation documentation..

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: On the lower level of the facility a deodorizer spray was in an unlocked cabinet underneath a sink. The kitchen area was accessible to children and had cleaning materials underneath the sink that was not locked. The older infant room had a disinfectant spray that was on a high shelf, but was not locked away.

Description: Based on observation, a substitute container was not labeled to indicate their contents. Evidence: In the older infant room an unmarked bottle with clear liquid was stored with other cleaning agents, but was not labeled.

Description: Based on observation, all electrical outlets did not have protective covers. Evidence: On the lower level of the facility, one outlet was left uncapped near the changing table and another outlet was left uncapped on the back wall.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on observation, an employee who is alone with, in control of, or supervising one or more children did not have the required background check. Evidence: Staff #1 was alone with children on the lower level of the facility, but did not have successful background checks returned and on file. Staff member was hired on 10/5/2015 and is allowed to work, but under the direct supervision of an employee that has completed background checks on file.
