Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, October 20, 2015 from 11:00am to 1:10pm. The children were observed engaged in a variety of activities while they waited for lunch. The infants and toddlers were observed getting ready for lunch. The Twos were observed singing songs and listening to a story. The preschoolers were observed listening to the story, "Big Pumpkin". A formal observation was completed with the twos and preschoolers. Lunch consisted of fish sticks, broccoli, grapes, cheese bread and milk. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. All classrooms and playgrounds were inspected today as well as the facility?s van. Four children?s records and two employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill:10/2/2015 Last shelter-in-place drill: 10/2/2015 Last fire inspection: 6/4/2015 Last health inspection: 9/3/2015 Total enrollment: 75 Today, the following child to staff ratios were observed: Infants ? 4:1 Toddlers ?5:1 Twos ? 11:2 Older Twos-7:2 Preschool (Threes and Fours) ?11:2 School Age ?not present The violations from the previous inspection in April of 2015 were checked for corrections. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records on 10/20/2015, the center did not ensure that one of four records reviewed contianed a physical examination. EVIDENCE: The record of Child #3, whose first day at the center was 12/7/2014, did not contain documentation of a physical. Each child shall have a physical examination before the child's attendance; or within one month after attendance.

Plan of Correction: Will have parent bring it in today (10/20/2015)

Standard #: 22VAC40-185-330-B
Description: Based on observation on 10/20/2015, the center did not ensure that the required depth of resilient surfacing was kept around the playground equipment. EVIDENCE: Two of four depth samples taken around the slide equipment on the playground measured 4 inches and 4.10inces. The height of the equipment requires a minimum depth of six inches of resilient surfacing. Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard
