Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Comments:

An unannounced, on-site monitoring inspection was initiated on November 7, 2024 and completed on (November 7, 2024, as a part of the current licensure period. The on-site inspection began at 8:35am and ended at 9:15am. The inspector reviewed compliance in the areas listed above. There were 6 children present and2 caregivers; the point total was 18. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Description:	Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for children A and B did not have a parent?s signature for an annual review of the emergency contact information. Child A had been in care for over a year. Children B had been in care for over two years without any annual review.
Plan of Correction:	I will have parents sign the annual review of the information.
