Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Staff of the Chesterbrook Academy #801 located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 106 children with 19 staff; good ratios in all 7 rooms. A total of 3 children?s files and 2 new staff files were reviewed. The children were engaged in the following: outdoor play on the playground, circle time, song, story time, art, diapering, tummy time, hand washing, bathroom breaks, lunch time, nap time, and other organized activities. Standard #: 22VAC40-185-160-C Description: Based on review, 1 of 2 new staff files reviewed indicated (from the documentation on hand) that it has been more than two years from the date of the most recent tuberculosis (TB) screening or testing. Evidence - 1. On the date of inspection (10/27/2015), the most recent TB documentation on file was dated - 09/10/2012. 2. Other documentation was on file that listed various immunizations, but it was so faded that it could not be deciphered. Plan of Correction: Updated or clearer documentation will be obtained and placed on file.
Standard #: 22VAC40-185-70-A Description: Based on review, 1 of 2 new staff files reviewed indicated that not all of the required documentation was contained within each staff record. Evidence - 1. The two required references were not completed for Staff #2. Both forms were left blank. 2. The Director stated (during an interview conducted on the date of inspection - 10/27/2015) that she believed that she had at least one written reference on hand. 3. The start date for Staff #2 was listed as - 06/19/2015. Plan of Correction: At least two references will be completed and placed on file for Staff #2.
Standard #: 22VAC40-185-270-A Description: Based on observation, there were areas both inside and outside the center that were not maintained in clean and/or safe condition. Evidence - 1. On the date of inspection (10/27/2015), the inner soffit area in the covered walkway (outside the Toddler II's room) shows areas where the drywall tape has come away from the ceiling's surface; exposing the corner underneath; leaving a gap. 2. Outside the Preschool I's room, a screen was observed with shredded mesh, and a bent frame. 3. Inside the Toddler I and 2's rooms, the changing pad in each appears to be heavily worn. 4. Inside the Toddler II's room, the blue floor mats are frayed on each corner. Plan of Correction: All areas and equipment in need of repair will either be replaced, fixed, or if needed discarded altogether.
Standard #: 22VAC40-185-510-J Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (10/27/2015), the medication cabinet, located in the center hallway, outside of the kitchen door, was left unlocked. 2. None of the prescriptions were designated otherwise from orders written by a physician. 3. The cabinet was not in use at the time of this finding. Plan of Correction: The cabinet will remain locked at all times, except when obtaining or returning medication.
Standard #: 22VAC40-185-540-A Description: Based on observation, not all of the first aid supplies were kept on site. Evidence - 1. On the date of inspection (10/27/2015), all four tubes of activated charcoal preparation had expired. 2. One tube was dated - 09/2013, and the other three tubes were dated 05/2015. Plan of Correction: These expired items will be properly discarded and replaced.
