During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on November 18, 2024. The on-site inspection began at 10:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above.There were 8 children present and 2 caregivers; the point total was 25points. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site. The inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-780-B
Description: REPEAT VIOLATION
Annually, the provider shall review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
The emergency contact information for child #1 has not been reviewed for over 14 months.

Plan of Correction: We will make sure the children's records are review annually with the parents.

Standard #: 8VAC20-800-90-A
Description: Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.
The family day home had not obtained documentation that child #5 has been adequately immunized, who has been attending for over three weeks.

Plan of Correction: We will get the missing record. We will make sure to get an immunization record before a child's first day.

Standard #: 8VAC20-800-90-C
Description: The family day home shall obtain documentation of additional immunizations for a child who is not exempt from the immunization every six months and under the age of two.

Child #2 who is under the age of two has not had updated immunization documentation for over 20 months.
Child #3 who is under the age of two has not had updated immunization documentation for over 23 months.

Plan of Correction: We will get an updated record and make sure to get an updated record every six months for those under two years old.

Standard #: 8VAC20-800-140-B-3
Description: The provider is required to have a current certification in cardiopulmonary resuscitation (CPR).

Certification for CPR for the provider has been expired for over six weeks.

Plan of Correction: I will make sure my CPR is current

Standard #: 8VAC20-800-140-B-4
Description: The provider is required to have a current certification in first aid.

Certification for first aid for the provider has been expired for over six weeks.

Plan of Correction: I will make sure my first aid is current

Standard #: 8VAC20-800-170-A
Description: REPEAT VIOLATION
The provider shall obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form.
Caregiver #1 has been in contact with children for over 18 months without obtaining a current report of tuberculosis screening.

Plan of Correction: We will make sure caregivers have TB before they come in contact with the children.

Standard #: 8VAC20-800-180-A
Description: REPEAT VIOLATION
The provider shall obtain for each caregiver a current Report of Tuberculosis (TB) Screening form, every two years from the date of the first screening .
The current TB screening for the provider has been expired for over 8 weeks.

Plan of Correction: I will make sure TB is current for all caregivers.

Standard #: 8VAC20-800-320-G
Description: REPEAT VIOLATION
Wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider.
The wood burning stove and associated chimney have not been inspected annually.

Plan of Correction: We will get the fireplace and chimney inspected annually.
