Comments:
A complaint inspection was conducted today from 8:25am-9:35am regarding an allegation of staff to child ratios not being met. Classrooms were observed, staff interviewed, and documentation reviewed. Based upon review of documentation, a preponderance of evidence supports the allegation, thus the complaint is deemed valid. Areas of non-compliance are identified in the Violation Notice. Complaint related: Yes
Description: Based upon record review, the center did not ensure that there was always one staff member for every 10 children for children three years through five years of age. Evidence: 1. Upon review of the Child Supervision Record (CSR), on October 19, 2015 at 8:00am there was one teacher supervising a total of 13 children in the Pre-K classroom, thus exceeding the 1:10 staff to child ratio. 2. Upon review of the CSR, on October 23, 2015 at 8:00am there was one teacher supervising a total of 11 children in the Pre-K classroom, thus exceeding the 1:10 staff to child ratio. 3. Upon review of the CSR, on October 26, 2015 at 8:30am there was one teacher supervising a total of 11 children in the Preschool classroom and one teacher supervising a total of 13 children in the Pre-K classroom, thus exceeding the 1:10 staff to child ratio. 4. Upon review of the CSR, on October 28, 2015 at 8:00am there was one teacher supervising a total of 12 children in the Pre-K classroom, thus exceeding the 1:10 staff to child ratio. 5. Upon review of the CSR, on October 29, 2015 at 8:30am there was one teacher supervising a total of 12 children in the Pre-K classroom, thus exceeding the 1:10 staff to child ratio. The CSR observed for both the Preschool and Pre-K classrooms were not documented accurately. Staff signatures were observed on the back of the form to document the times that staff sign in/out. However, on the front of the form, all staff initials were not observed documented and the time in which the classrooms were not in compliance with staff to child ratios was observed.

Plan of Correction: I will retrain staff by the end of the week.
