An unannounced complaint inspection was initiated on October 20, 2015 in response to a complaint regarding Staffing and Supervision, Programs and Special Care Provisions and Emergencies. Interviews were conducted with the licensee and staff . Licensing inspector reviewed Staffing and Supervision, Program and Special Care Provisions and Emergency . Based on interviews, inspection of the facility and a review of records and documentation, there was not a preponderance of evidence to support the allegation therefore the complaint is deemed NOT VALID. Violations were found that were not reported, please see Violation Notice for further details. Exit interview was conducted with Program director.

Description: Based on record reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on October 20, 2015, the record for child #1 ( start date 8/10/15) did not have a physical examination.

Plan of Correction: We have taken steps to prevent this from happening in the future.

Complaint related: No
Description: Based on injury report reviewed, the licensee did not ensure there was written record of all required information for injury reports. Evidence: During the inspection conducted on October 20, 2015, injury report for child #1 from August 27, 2015 did not have documentation of future action to prevent reoccurrence of injury, and documentation of the time parent was notified.

Plan of Correction: Remind all staff to fill out all sections of the report.
