Comments:
An unannounced non mandated inspection was completed today 11/4/2015. There were 14 children in care on the day of the inspection with three staff. There were four children's and three staff records reviewed during the inspection. A physical plant walk through was completed and observations of the children. Discussion: about aides and using them for bathroom breaks, using all classroom, and records.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of four children's records, the center failed to ensure that all records had a physical with in 30 days of enrollment. Evidence: 1. The inspector found that child # 3 did not have a physical on the day of the inspection. The start date for the child was 9/28/2015.

Plan of Correction: The director will obtain a copy from the parents and will monitor all new enrollments to ensure time lines are followed.

Standard #: 22VAC40-185-540-A
Description: Based on observation of the vehicle's first aid kit, the center failed to ensure that the kit was complete. Evidence: 1. The inspector found that the van's first aid kit was missing the following: antiseptic and two triangular bandages.

Plan of Correction: During the inspection a staff person placed the missing items in the first aid kit and all kits will be monitored.
