An unannounced monitoring inspection was conducted from 10:15 a.m.-12:30 p.m. with the administrator, owner, and six staff. 44 children, ranging in age from 2 years to 5 years old, were in the care of eight staff. Children were observed drawing rainbows, painting with orange and red paint, playing Zingo, reading books, and washing hands to prepare for lunch. Lunch served today was chicken nuggets, mashed potatoes, broccoli, applesauce, and milk. The health and fire inspections are both current. The battery operated radio and flashlights were present and accessible. The last fire drill was 10/14/15. The last shelter-in-place drill was 10/15/15. Two staff records were reviewed. Standard #: 22VAC40-191-40-D-1-C Description: Based on review of two staff records, the center did not have sworn disclosure statements that were updated every three years. Evidence: Staff A's last signed sworn disclosure statement was dated 9/18/2012. Plan of Correction: Staff A signed a sworn disclosure statement during the inspection.
