The Licensing Inspector talked with the Director about the annual fire inspection.
An unannounced monitoring inspection was conducted on 11/19/15 from 9:40am - 10:45am. During the inspection there were 11 children ages one to five years old in care with five staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms. Records were reviewed for three children and three staff. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were reviewed during the exit interview. lease complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-160-C Description: Based on a review of three staff records, it was determined that the facility did not ensure that staff resubmit updated TB results every 2 years. Evidence: 1. The record for staff #2 had documentation of a tuberculosis screening dated 8/30/13. 2. The record for staff #3 had documentation of a tuberculosis screening dated 8/1/12. 3. Staff #4 confirmed that an updated tuberculosis screening had not been received for staff #2 or staff #3. Plan of Correction: Staff #2 and staff #3 will get a TB screening.
Standard #: 22VAC40-185-280-B Description: Based on observation, it was determined that the facility did not ensure that all hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. There was a can of air freshener on a shelf by the sink in the Toddler classroom. The label contained the statements "keep out of the reach of children" and "flammable". 2. Staff #1 confirmed that the air freshener should be stored in a secure location not accessible to children. Plan of Correction: Corrected during the inspection. Staff placed the air freshener in a locked closet. All hazardous items will be stored in a locked place.
Standard #: 22VAC40-185-550-E Description: Based on a review of the emergency drill log, it was determined that the facility did not ensure that a record of the emergency evacuation drills were maintained for one year. Evidence: 1. On the emergency drill log, the most recent emergency evacuation drill documented was dated 9/28/15. The space where the emergency evacuation drill for October 2015 should be recorded was blank. 2. Staff #2 stated that they had completed an emergency evacuation drill during the month of October 2015, but could not give a specific date or provide documentation that it had been completed during the inspection. Plan of Correction: The emergency evacuation drills will be recorded on the emergency evacuation log.
