Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding the requirement of observing children in care.

Comments:
An unannounced monitoring inspection was conducted today, November 20, 2015 to determine the center's compliance with licensing standards. No children were in care during this inspection as a result standards regarding supervision could not be determined. All areas used by children were inspected to include the main hallways, children's restrooms, classroom, and office/child area. No hazards were found on this day.

The center had the required postings displayed on the wall upon entering the classroom. The center's first aid kit and emergency supplies were inspected with the required supplies found. Two children's and one staff records were inspected with the majority of the required information present, see attached notice. The following was also inspected on this day: daily schedule, annual fire & health inspections, fire/shelter-in-place drills, injury prevention plan, parent agreements, and other pertinent information

Description: Based on the inspection of one staff record, the required sworn disclosure form expired.

Evidence:
During the inspection of one staff record reviewed, it was discovered the sworn statement or disclosure form dated 8/4/12 expired on 8/3/15. This form was supposed to be updated prior to expiration, all other background checks are current.

Plan of Correction: Per director: I will complete the form and place it in the record.
