22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 10:15am-11:45am. Upon arrival there were 52 children in care with 11 staff present. A sample of 2 staff and 4 children's records were reviewed in addition to 4 medications. Children were observed on the playground and in learning centers. Lunch preparation was also observed.
Standard #: 22VAC40-191-60-B Description: Based on a record review the center failed to ensure employees had a completed worn statement of affirmation prior to their hire date. Evidence: Staff #2 was hired on 09/21/15, however the sworn statement of affirmation in the file was dated 09/22/15. Plan of Correction: This was an oversight as staff do an orientation with Sentara prior to arriving at the center. The director will ensure a sworn statement is assigned before an employee starts in the future.
