Technical Assistance:
Discussed assigning a staff person to monitor the playground daily before the children go on it.

Comments:
Unannounced monitoring inspection was conducted beginning at 11:30 and ending at 12:30. The inspection was done to check issues on the playground. The resilient surface on the playground had been fixed however their was an issue with the black gate again, The next inspection will be in January for your Renewal Inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation the the playground, it was determined that their was a section of the fencing on the gate that was unsafe, Evidence: The bottom of the gate had a sharp metal piece at the bottom of the gate had come undone, leaving a sharp end exposed and accessible to the children.

Plan of Correction: The Director will notify the maintenance staff today and will have a staff person assigned to inspect the playground daily for safety issues.
