Comments:
An unannounced monitoring inspection was conducted on November 19, 2015 from approximately 3:35 PM to 4:30 PM. Two staff were present with 29 children. Children were observed playing on the playground, preparing for snack, playing on computer, building, arts crafts and board games. Three children's records were reviewed and medication documentation was reviewed on this date.

Violations:
Standard #: 22VAC40-185-510-N
Description: Based on observation and a medication review, licensee did not ensure that when an authorization for medication expires , the parents shall be notified the at the medication needs to be picked up with in 14 days or parent must or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center .

Evidence:
During the inspection conducted on November 18, 2015, there was a medication with expired authorizations and medication was not returned to parent or disposed of by the center. The medication authorization for Child #1 expired on September 24, 2015.

Plan of Correction: A calendar will be created to track expiration dates.

There was an authorization form with the one mentioned which expired 9/24/15. It was for the same child and medication.

Standard #: 22VAC40-185-540-A
Description: Based on observation, the licensee did not have the items for a a required first aide kit. Evidence : During the inspection conducted on November 18, 2015, there was no tweezers in the first aide kit.

Plan of Correction: Multiple pairs of tweezers will be purchased to ensure there is always a back up pair.
