An unannounced monitoring inspection was conducted today, November 13, 2015 to determine the center's compliance with licensing standards. Most but not all standards were reviewed. The licensing inspectors arrived at approximately 10:00 am and concluded the inspection at 12:00 pm. The children were engaged in a variety of activities upon the inspectors arrival. Children in the preschool classrooms were having center time. In the art center, children were creating Native American headbands by decorating paper headbands with colorful paper shapes, markers and feathers. Children then choose an Native American name for themselves , one child chose " Chief Pink Unicorn". Other centers included lego table, trains/cars/tracks, house keeping, writing and listening center with books and tapes. Each classroom had added a teepee to the home living area. Children looked at books while in the teepee and pretended to be native Americans with paper towel tubes & tissue paper camp fires. The pre-kindergarten children painted leaves and created leaf prints. Classrooms were decorated with seasonal decorations and themes of Thanksgiving carried over into centers and activities. In the two year old classroom, children worked on their individual journal books (notebooks, that the children colored pictures, wrote letters, name and glue pictures from magazines, photos, and scrapes of leftover materials in). Children also played with bean bags and scarfs to music. Toddler were observed singing "10 Little Indians". Diaper and preparation for lunch with hand washing were observed. Staff interacted, encouraged children and assisted with activities. The inspector interacted with children and staff during the inspection. The classrooms have a variety of age and stage appropriate materials that are in operable condition. All classrooms were inspected to include the front entrance, kitchen and the cafeteria. The play grounds were not inspected as outdoor building maintenance was being conducted during the inspection on the playground. Additional pea gravel had been installed. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The first aide kit and the emergency supplies were inspected and found to be complete. Two employee records and four children's records were reviewed during today's inspection. Medication is being administered. Medication and medication authorizations were reviewed. There are multiple staff with current MAT training, CPR, First aid and daily health observation training. Current Health Inspection: 02/09/2015 Current Fire Inspection: 09/11/2015 The last emergency evacuation drill was conducted on 10/29/2015, The two required shelter-in-place drills for the 2014 year were documented on 02/21/2014 and 12/16/2014. A shelter-in-place drill for 2015 was documented on 03/17/2015. The emergency preparedness plan had been reviewed/updated. Today the following ratios were observed: Toddler Room: 7 toddlers with 2 staff. 2's Nursery: 10 children with 2 staff. 3's Pre-School: 14 children with 3 staff. 4's Pre-K: 13 children with 3 staff. Jr. Kindergarten: 8 children with 1 staff. School age: Not in attendance during inspection. The monthly menu was posted and today's lunch consisted of: steak nuggets, mixed vegetables, pineapple, roll and milk. The violations from the previous inspection on September 21, 2015 were checked for corrections. There were no repeat violation cited during today's inspection. Standard #: 22VAC40-185-60-A Description: Based on review of children's records, the facility did not ensure all required information was obtained. Evidence: The record of child #3 did not have the name, address and phone number for a second emergency contact as required. Plan of Correction: Per the Director: I will contact the parent tonight at pick up and have them complete the missing information in the child's record.
Standard #: 22VAC40-185-510-E Description: Based on of medications, the facility did not ensure medications were labeled with the child's name, the name of the medication, the dosage, and the time or times to be given. Evidence: The prescription medication for child #5 was not in its original container and was not labeled with all the required information. Plan of Correction: Per the Director: I will have the parent provide the original container with all the required information listed on it to be kept with the medication.
