Standard #: 8VAC20-800-800-B Description: The provider shall review the emergency plan at least annually and update the plan as
needed. The provider shall document in writing each review and update to the emergency
plan. The most recent review and update of the emergency plan was documented on November 30, 2023.Plan of Correction: I will review the emergency preparedness plan today and document that review with my signature and date.
Standard #: 8VAC20-800-60-B Description: A child's record shall include the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached, as well as documentation of an annual review of the child?s emergency contact information.
The records for Child #1 and Child #2 were missing required information for one of the two designated emergency contacts. Additionally, the records for Child #1 did not contain documentation of an annual review of emergency contact information, despite the child attending the family day home for over 13 months.Plan of Correction: I will have the parents send me the required information to add to the children's records.
