Technical Assistance:
Timing of snacks and lunch was discussed. There must be at least 1 1/2 hours but no more than 3 hours between snacks and lunch.

Comments:
The inspector arrived at 8:15 AM, inspected the outdoor playground, and arrived inside at 8:25 AM. The inspector departed at 11:10 AM. Upon arrival there were three children in care with two staff. Morning arrival, activities and snack service were observed. A sampling of five staff records and five children's records were reviewed. It was reported that no medication has been administered to children and no emergency medication is stored at the facility. No children's injuries were reported. An exit meeting was conducted with the program director, during which the facility's plans of correction were obtained, prior to conclusion of the inspection. Violations:
Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records, the facility has not ensured that each child's record contains documentation of viewing proof of the child's identity and age. Evidence: The record for child 2, who began attending on 10/5/2015, does not include documentation that the child's identity and age was verified.

Plan of Correction: The facility responded with the following: The parent will be contacted to bring the birth certificate back in for the child's record. A check will be made of all children's records to ensure that they include documentation of the proof of identification.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by chidlren. Evidence: Upon arrival of the inspector at 8:25 AM, there were three children in care in the multi-purpose room. The door that allows direct access into the kitchen from the multi-purpose room was not closed securely and locked. The door into the kitchen fro the hallway was also unlocked. There was a container of aloe vera gel, labeled "warning, keep out of reach of children" on the counter by the window between the kitchen and the multi-purpose room. In an unlocked cabinet under the kitchen counter there were numerous and various cleaning products, such as Comet Cleanser and Lysol Multi-Purpose Cleaner, with warning labels to include to keep out of reach of children.

Plan of Correction: The facility responded with the following: Morning opening staff will be reminded to check both doors into the kitchen to ensure they are locked prior to the arrival of the children. All staff will be reminded that the kitchen must be locked after use. Staff will be required to lock the door behind them when they go out. Staff will also be reminded at our next staff meeting.

Standard #: 22VAC40-185-530-A
Description: Based on observation and staff interview, the facility has not ensured that there is at least one staff member trained in first aid and cardiopulmonary resuscitation (CPR) on the premises during the center's hours of operation. Evidence: According to the director, staff 8 is the only staff person with current first aid and CPR certification. The facility is open from 8:00 AM until 4:00 PM. Staff 8 works from 9:00 AM until 1:00 PM only. From 8:00 AM until 9:00 AM and from 1:00 PM until 4:00 PM there is no staff person with current certification.

Plan of Correction: The facility responded with the following: Staff will be scheduled to obtain first aid/ CPR certification as soon as it can be scheduled.

Standard #: 22VAC40-185-560-F
Description: Based upon observation, the facility has not ensured that there is a menu listing foods to be served for snacks during the current one-week period and that the menu is kept on file for one week. Evidence: The posted pre-K (room 1) snack menu has only one food group indicated for Tuesday, Wednesday and Thursday of last week and no food indicated for the other days last week or for this current week's snacks. The posted Toddler (room 3) menu has only one food group indicated for Wednesday and Thursday of last and no food indicated for the other days last week or this current week's snacks. The posted menu for the current week for room 5 shows only one food group component instead of the required two food group components.

Plan of Correction: The facility responded with the following: All classrooms in which snack is served will post the current week's snack menu to include at least two components and staff will be required to keep a file of the menus that have been posted.
