An unannounced inspection was conducted from 12:00pm to 1:20 pm. At the time of entrance, there were 43 children present with 5 staff present and 1 administrator. The inspector observed children participating in lunch, transitioning between activities, nap/ quiet time, toileting and hand washing procedures. A sample size of 5 children's records and 4 staff records were reviewed, in addition to medication and injury reports. Areas of noncompliance are identified in the Violation Notice and were discussed with the Director at the conclusion of this inspection.
Standard #: 22VAC40-185-90--A Description: Based on record review it was determined that the center did not ensure that all written agreements between the parent and the center were in each child's record by the first day of attendance as required. Evidence: The record for child #2(enorlled 9/8/15) did not contain the following signed parental agreements: -An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately -A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center -A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately Child #2 was in care on the day of the inspection. Plan of Correction: Director will have the mother sign the required parental agreements at pick up.
Standard #: 22VAC40-185-550-M Description: Based on record review it was determined that the center did not ensure that all written records of children's serious and minor injuries contained all of the required components. Evidence: The injury report for child #6 did not contain the time of parental notification. Plan of Correction: Director will speak with staff about completing injury reports.
