An unannounced complaint investigation was conducted on 10/14/15, 10/15/15, 10/19/15 and 11/05/15 in response to a complaint received by the licensing office on 10/13/15. Interviews were conducted relating the allegation of behavioral guidance. The information gathered during the investigation does support the allegation, so the complaint is determined to be "valid". There were also a violation in the area of children's records found during this inspection. If you have any questions, contact the licensing inspector at (757)354-7307.

Description: Based on a record review it was determined that the center did not ensure that each child's record contained all of the required components. Evidence: 1.The record for child#1 (enrolled 3/31/15) did not contain any documentation of her mental health diagnosis and any special accommodations needed. 2. It was reported by the center staff that child #1 is diagnosed with a mental health condition for which she requires medication management and intensive in home therapy. 3. The center staff stated that they were made aware of the above by the child's parent after child #1 had her first significant behavioral episode on 5/8/15. 4. The center utilizes a abbreviated time in care and assistance from the in home therapist in the center to try to maintain the child in care however; this was not documented in the record.

Plan of Correction: Any child who enrolls into the Boys & Girls Clubs and attends an alternative school must go through an orientation along with their parent or guardian before enrolling. Prevention Method: After any incident the center will document in writing on the Boys & Girls Clubs incident/accident report form and also on the center?s membership forms. Medication not noted by the parent after finding out the child requires medication management and any health issues not noted by the parent will be documented on the center?s form. The center will make note on any child?s application if an in home care therapist or case worker is assigned to a child.

Description: Based on interviews it was determined that the center did not ensure that all behavioral guidance is constructive in nature, age and stage appropriate, be intended to redirect children to appropriate behavior and resolve conflicts. Evidence: It was reported by staff #1(hired 5/30/07) that: -Child #1 hit another peer. Staff #1 asked child #1 to leave the room. Child #1 refused to leave the room. Staff #1 placed her hand on child #1's shoulder to guide her, child #1 then slid from her chair onto the floor. Child #1 swung at staff #1 with a pencil, then tried to bite and kick staff #1. Staff #1 then held child #1's ankles and slid across the floor with the child by the ankles to remove her from chairs and the other children. Staff #1 sat remained with child #1 until staff #2 arrived in the classroom and was able to have child #1 leave the classroom willingly. -Staff #1 did not provide appropriate behavioral guidance to child #1 in order to redirect to appropriate behavior and resolve conflicts.

Plan of Correction: Prevention Method: Boys & Girls Clubs will ensure proper training of center?s staff which will include conflict resolution training and provide informal guidance and discipline training techniques through Boys & Girls Clubs University. Appling these techniques and strategies while working at the Club will help staff recognize a set of informal guidance techniques to support positive behavior and discipline strategies to hold youth of all ages accountable for their behavior.
