Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated and completed on January 15, 2025. The on-site inspection began at 10:00am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 17. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-800-60-B
Description:	A child's record shall contain the name, address and telephone number for two emergency contacts in the event a parent or guardian cannot be reached in an emergency. Child #1 who had been in care for over one year does not have the name, address and telephone for a second emergency contact.
Plan of Correction:	I will contact the parent and get the information for the second emergency contact. I will make sure to get the information for both emergency contacts before the child starts.
Standard #:	8VAC20-800-680-A
Description:	REPEAT VIOLATION Caregivers shall wash their hands with liquid soap and warm running water after each diaper change. Caregiver #2 did not wash their hands after assisting child #1 and #2 with diapering.
Plan of Correction:	We will make sure hands are washed after diapering.

Description:	Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering. Child #1 and #2 did not have their hands washed after diapering.
Plan of Correction:	We will make sure hands are washed after diapering.
Standard #:	8VAC20-800-690-D
Description:	The diapering surface shall be cleaned and sanitized after each use. The diaper surface was not cleaned and sanitized after each diaper change.
Plan of Correction:	I will speak with the other caregiver to ensure the diapering surface is cleaned and sanitized after each use.
