An unannounced, on-site monitoring inspection was initiated on January 15, 2025 and completed on January 15, 2025, as a part of the licensure period. The on-site inspection began at 12:29pm and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There were ten children present and two caregivers; the point total was twenty-nine. The inspector reviewed five children?s records and three caregiver/household member(s) records on-site. This inspection included document review and a tour of the family day home.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.036-A Description: The provider is required to obtain a competed sworn statement from adults living in the family day home every five years.
The most recent sworn statement for Household Member #3 was overdue by twenty six days.Plan of Correction: Will have the household member complete the sworn statement form today.
