Comments:
An unannounced, on-site renewal inspection was initiated completed on 1/16/2025. There were 11 children present and 2 caregivers. The inspector reviewed 6 children?s records and 3 caregiver/household members records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), a tour of the family day home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-120-B
Description: Caregiver records shall contain all required information.

Caregiver #3s record did not contain documentation of two references.

Plan of Correction: I did verbal references, I didn't know I need to write them down. I'll complete the forms tonight.

Standard #: 8VAC20-800-530-A
Description: Cribs, cots, rest mats, and beds when being used for sleeping or napping by children other than infants shall have linens consisting of a top cover and a bottom cover.

Four out of nine children napping did not have a bottom cover/linen.

Plan of Correction: I have extra sheets but didn't know the children on cots needed them. I'll start using them tomorrow.
