Comments:
An unannounced inspection to renew the license was conducted on December 11, 2015 from 9:54 a.m. to 1:23 p.m. Upon arrival, there were 28 children ages 2 years to 6 years old with 6 staff present. Records were reviewed for 4 children and 3 staff. There was 1 medication review. During the inspection, children were observed engaged in outdoor play, independently looking at books, individual choice play, assembling puzzles, taking photographs with Santa, eating lunch and resting. Areas of noncompliance were found in special care and emergencies and special services. The violations are identified on the violation notice and were discussed with the director during the exit interview.

Violations:
Standard #: 22VAC40-185-550-C
Description: Based on observation. the facility did not ensure that emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: 1. Emergency evacuation and shelter-in-place procedures were not posted with the maps. 2. The director confirmed this.

Plan of Correction: The director will post the procedures by 12/19/15.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the facility did not ensure that when food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence: 1. One (1) of four (4) lunch bags, in the classroom with 4 and 5 year old children, had no name and date on the clear plastic bag, with crackers inside, or on the lunch bag. 2. Staff confirmed that the owner's name and the date were not on the bags.

Plan of Correction: We will send out another reminder to all parents and attach an additional note to the parents that are not folliwing the rules.
