Comments:
An unannounced monitoring inspection was conducted on December 10, 2015 from approximately 7:10 AM to 9:45 AM. Two staff were present with nineteen children. Children were observed participating in table activities. Three children?s records and two staff records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Description: Based on review, facility did not ensure 1 of 3 children's records has a physical examination within 30 days of beginning attendance. Evidence: On December 10, 2015, the record for child #1 (start date September 20, 2015) does not have a physical.

Description: Based on review, facility did not ensure 3 of 3 children's records have documentation of required information. Evidence: On December 10, 2015, the following was found: 1. The record for child #1 (start date September 20, 2015) did not have documentation of emergency contact addresses. 2. The record for child #2 (start date September 8, 2015) did not have documentation of father's employment phone number. 3. The record for child #3 (start date September 8, 2015) did not have documentation of second emergency contact address.
