22VAC40-191 Background Checks (22VAC40-191)
Today an unannounced monitoring visit was conducted beginning at 11:15am. There were 13 children present with 3 teachers and the Director. Children were observed playing outside on the playground and eating lunch. Today there is an enrichment program for children about health and emotions. Children have been creating crafts such as mangers and bells for the holidays. Seven staff records, 5 children's records and 3 medications with authorizations were reviewed today. Standard #: 22VAC40-185-60-A Description: Based on record review, one child's record (#3) is missing complete required documentation in their file. Evidence: Child #3 has a start date of 9/8/15 and there is no documentation of a physical examination within 30 days of their start date. Plan of Correction: We will ask the parents to provide us with this documentation.
Standard #: 22VAC40-185-510-G Description: Based on direct observation, one medication present for a child (#11) does not have a prescription label on the medication from a pharmacy. Plan of Correction: We will ask the parents to provide the medication with a prescription label.
Standard #: 22VAC40-185-560-G Description: Based on direct observation and discussion, food brought from home is not labeled with children's names and the date. Evidence: Children's lunch boxes/bags do not all have a name on them indicating the owner. Food containers brought from home also do not have the child's name on them or the date. Plan of Correction: We will send out a letter to parents telling them that we will begin requiring this when school starts again in January.
