Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated on 1/27/2025 and completed on 1/27/2025 as part of the conditional licensure period. The on-site inspection began at 9:30 am and ended at 10:40 am. The inspector reviewed compliance in the areas listed above. There were (11) children present and (4) caregivers; the point total was (35). The inspector reviewed (5) children?s records and (2) caregiver/household member records. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-800-60-B
Description:	Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency. The record for Child #4 was missing a second name, address, and phone number of a designated person to contact in case of an emergency. The child was present during the inspection and had been in care for 4 months.
Plan of Correction:	Parent will provide the information.
Standard #:	8VAC20-800-70-A
Description:	Before the child's first day of attendance, parents shall be provided required information in writing.' The parents of Children #2, and #3 were not provided in writing the following required information: provisions of the family day home?s emergency preparedness and response plan. Children had been in care for three months.
Plan of Correction:	Parent s will sign the policy and be filed in the child's record.

Description:	Before attendance, documentation of immunizations is to be obtained for each child. There was no documentation of immunizations for child #4, who was in care during the inspection and had been in care for 4 months.
Plan of Correction:	Parent will provide the current immunization record.
