Comments:

An unannounced, on-site renewal inspection was initiated and completed on 01/27/2025, as a part of the licensure period. The on-site inspection began at 10:30am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 19. The inspector reviewed 4 children?s records and 6 caregiver/household members' records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-800-780-B
Description:	REPEAT VIOLATION Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Record for child #1, did not have a parent?s signature for annual review of the emergency contact information. Child #1 has been in care for over a year.
Plan of Correction:	Provider to review information with parent and have form signed at pick up today.
Standard #:	8VAC20-800-100-A
Description:	The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child #1, who had been in care for over a year.
Plan of Correction:	Provider to request and obtain physical exam from parent at pickup today.
