Technical Assistance:
Consultation today included bathroom passes; and outdoor physical environment.

Comments:
An unannounced monitoring inspection was conducted today from 10:40 - 12:00 pm. There were two classes present with 30 children and five staff. I observed children working with manipulatives; cutting; involved in role-playing; celebrating a birthday and going out to the playground for the first time after several rainy days. Medications and records were well-organized and complete. If you have any questions, please contact me at brenda.kuhlman@dss.v.irginia.gov

Violations:
Standard #: 22VAC40-185-270-A
Description: During today's inspection, some areas were observed not to be maintained in a safe condition. Evidence: A hallway mat to the outdoors area had rips in the binding, posing a tripping hazard. There was a walkway wooden board that was observed to have become detached from the rest of the wooden walkway. Some large, splintering pieces of mulch and rock were observed in the fall zone of the slide structure.

Plan of Correction: The hallway mat was rolled up and removed during the inspection. The walkway will be repaired. The sharp pieces of wood mulch and rock were removed.
