We discussed the requirements for children's records. The risk assessment for a violation was provided. Reminders were provided regarding the information that must be posted in view at all times. You were provided recommendations regarding the use of the bulletin board. We talked about the documentation of the time of parent notification for minor injuries treated by staff.
Thank you for your assistance during the unannounced inspections on December 8, 2015 from 3:10 PM to 5:50 PM at the school site and on December 10, 2015 for a staff record review in the main office. On December 8, 2015, there were twenty-one children with two to three staff. It was reported that there were twenty-one children supervised by two staff during before school hours this date. I viewed program activities, supervision, staffing, interactions with children, equipment, agency inspection reports, insurance, daily routines, snack, five records for children, five staff records, staff qualifications, staff training, five records for children, emergency supplies and drill records, injury records, posted information, indoor and outdoor areas. The children were provided homework assistance, a holiday coloring project for a display in the cafeteria, and active play activities on the back playground. Adequate supplies were available for table activities. I observed improved communication between staff and the assurance of supervision during transitions such as while individual children were returning to the cafeteria after they completed their homework. Standard #: 22VAC40-185-140-A Description: Based on a review of five records for children, and an interview with administrative staff, the complete physical examination records for child 4 and 5 were not obtained as required within the first thirty days of attendance. Evidence: Child 4 enrolled on August 10, 2015 and the physical examination record was obtained late on September 23, 2015. There were two immunization reports and no physical examination information in the file for child 5 who began attending on August 18, 2015. Plan of Correction: The team leader/site director immediately obtained a copy of the physical examination report for child 5 from the school office staff. The team leader/site director will review the records of children enrolled in 2015 and make sure they contain all required information. The administrative staff will assist the team leader with a reminder system for ensuring that the physical examination records are obtained within thirty days of enrollment.
