Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Description:	The caregivers are required to have current certification in CPR. Staff A had no current documentation of current CPR. The previous CPR expired over 4 months prior.
Plan of Correction:	I will get a current CPR certificate.
Standard #:	8VAC20-790-250-D-2
Description:	Caregivers shall have current certification in first aid. Staff A had no documentation of a current first aid certificate. The previous first aid expired over 4 months prior.
Plan of Correction:	I will get the current first aid certificate.
Standard #:	8VAC20-790-250-F
Description:	Caregivers shall annually complete the department's health and safety update training. Staff D had no documentation of a current health and safety training. The last completed training was over a year prior.
Plan of Correction:	Staff D will complete the health and safety training.
