22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today at 10:35am. There were 39 (infants-5yrs.) directly supervised by 10 staff and co-oping parents. The physical plant, 1 staff record, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, creating holiday art and playing outside. Hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found that CH1 and CH3 did not have documentation of an address for either of their emergency contacts.

Plan of Correction: Parents completed addresses for their emergency contacts.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the facility failed to ensure that all electrical outlets have protective covers. Evidence: Observed in Room 8, 2 uncovered electrical outlets - 1 underneath a clock and the other along the blue wall.

Plan of Correction: Electrical outlets were covered. Reminders to teachers to check every morning.

Standard #: 22VAC40-185-550-E
Description: Based on review, the facility failed to maintain documentation of the dates of drills practiced. Evidence: the facility failed to document whether or not they had completed a drill in November of 2015.

Plan of Correction: Document completed and posted on board and in file.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the facility failed to ensure that food brought from home is dated with the current date. Evidence: 1) Observed in Room 8, 7 lunch boxes without the current date. 2) Observed in Room 8, 1 lunch brought from home without the date.

Plan of Correction: Reminder to cooping parents to check the lunch boxes as they come in the classroom daily. If no date, add date after inspecting lunch box.
