Comments:
An unannounced monitoring visit was conducted today. All classrooms were in ratio with direct care staff. Children were observed playing outside and transitioning into the center, free play in classes (play dough, blocks, dramatic play and art) as well as infant care, letter writing, transition to lunch and rest time. Lunch served today was turkey tacos, corn, cheese and fruit as well as milk. Emergency medication is on-site with trained staff, emergency preparation is current and up to date postings and permits. Areas of non-compliance are identified in this report. Please sign and date the report and return it to me within 10 calendar days from date of receipt. Standard #: 22VAC40-185-60-A
Description: Based on record review, an action plan for child #1 is not able to be implemented as the medication was not on-site. Evidence: The action plan for child #1 called for a medication to be given if an allergy reaction occurs. The center would not be able to administer the antihistamine as the medication is not on-site.

Plan of Correction: MEdication was brought with authorization.

Standard #: 22VAC40-185-290-3
Description: Based on observation, electrical outlets in two classes were missing protective covers. Evidence: An outlet in the Infant Two class room and an outlet in the Three's class were missing protective caps.

Plan of Correction: Outlet caps were replaced during the inspection by staff.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, a written authorization for a child was expired. Evidence: The action plan signed by the parent and physician giving the center the authorization to administer a medication expired 8/2015.

Plan of Correction: Written authorization received from the parent for 10 days while the physician completes the authorization.
