22VAC40-185 STAFFING AND SUPERVISION.
A joint investigation was conducted at the site following a self-report. Records were reviewed and staff were interviewed. Standard #: 22VAC40-185-270-A Description: During today's inspection, some areas of the center were observed not to be maintained in operable condition. Evidence: The door exiting from the first classroom (Classroom #1) did not latch securely when closed today; and the metal gate closure on the outdoor fence to the playground did not provide a secure closure today as intended. Standard #: 22VAC40-185-340-A Description: When staff were supervising children on 8/12/15, they did not ensure the care, protection and guidance of one child when that child was found on his own in the playground after his class returned to their room. Evidence: There is not a preponderance of evidence to substantiate how Child # 1 ended up on the playground without supervision. Staff report that two children were moved from Class #1 to Class # 2. Documentation of this, which is part of the program's "transition box" policy, was not completed for that time; and attendance on the program record was not done at 5:00 pm as is usual procedure. At approximately 5:20, Staff # 3 reported going outdoors to retrieve a child's missing object. While there, she found Child # 1 playing in the mulch. Plan of Correction: Not available online. Standard #: 22VAC40-185-340-F Description: Through staff interviews and record review, it was determined that a child (age 4) was not within sight and sound supervision when he was found alone on the playground. Evidence: According to staff interviews and record review, Child # 1 was unaccounted for approximately 10 - 20 minutes on 8/12/15. The child was found playing by himself on the playground by Staff # 2 at approximately 5:20 pm. There has not been a preponderance of evidence to determine if Child #1 did not enter the school with his class after they were on the playground; or if he re-exited from the building on his own after the class returned to the classroom. Standard #: 22VAC40-185-350-F Description: While children may be temporarily reassigned to another class for reasons of administrative necessity, staff report this has been a regular occurrence after 4:00 pm this summer. Evidence: Staff report and documentation confirms that children have been brought to other classes as numbers drop. On 8/12.15, Staff # 1 reported that Child # 1 and another child were brought to another class to keep numbers in ratio. Plan of Correction: Not available online.
