Comments:
The licensing inspector conducted an unannounced monitoring inspection Monday, December 7, 2015. The children were observed engaged in a variety of activities ranging from playing outside on the playground to playing in learning centers. The infants were observed being fed and playing with toys during floor time. Interviews were held with staff throughout the inspection. Additionally, when appropriate the inspector interacted with children in each classroom. The center has two buildings on site, one for preschool and one for school age and preschool. Both buildings as well as all classrooms and playgrounds were inspected today. The facility?s mini buses were not inspected due to being detailed during the inspection. Five children?s records and seven employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/18/2015 Last shelter-in-place drill:7/27/2015 Last fire inspection: 10/14/2015 Last health inspection: 9/9/2015 Total enrollment: 300 Today, the following child to staff ratios were observed: Infants (ages 6wk-12m) Ducklings: 12:3 Infants (ages 12-16m) Chipmunks: 9:3 Toddlers (ages 16-24m) Beavers: 10:2 Twos (ages 2yr) Friendly Frogs: 11:3 Two-Three Year Room: Squirming Squirrels: 12:2 Three Year Room: Giggling Geese: 13:2 Three-Four Year Room: Rocking Rabbits: 19:3 Three-Four Year Room: Terrific Turtles: 18:2 Four-Five Year Room: Soaring Eagles: 20:2 Four-Five Year Room: Awesome Otters: 24:3 Four-Five Year Room (half day Pre-K): Fantastic Foxes: 5:1 Kindergarten Room: Raccoons: 13:2. School Age: not present The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. Standard #: 22VAC40-185-280-B
Description: Based on observation on 12/07/2015, the center did not ensure that all hazardous substances were kept in a locked place. EVIDENCE: A container of disinfectant wipes were observed unlocked on top of a cabinet in the three year old, Rocking Rabbits, classroom. the wipes were not within the reach of the children. Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: Corrected on site. the wipes were placed in a locked cabinet.

Standard #: 22VAC40-185-520-A
Description: Based on observation on 12/07/2015, the center did not ensure that expired over the counter skin products were kept at the center. EVIDENCE: Child #1 had diaper rash cream at the center. The cream expired on 10/2015. Over-thecounter skin products shall not be kept or used beyond the expiration date of the product.

Plan of Correction: Sending home the expired cream and will ask parent for new cream.
