An unannounced monitoring inspection was conducted from 1:15 p.m. - 3:15 p.m. with the center director and assistant director. 63 children, ages four months through 5 years, were in the care of seven staff. Children were observed having tummy time, reading a book, and having rest time. Lunch served today was turkey sausage and cheese, English muffins, roasted potato wedges, diced apricots, and milk. The health inspection is current. Three staff records were reviewed and are complete. Three children's records were reviewed. Five medications were reviewed and are in compliance. Standard #: 22VAC40-185-60-A Description: Based on review of three children's records, the center did not maintain complete children's records. Evidence: Child A's (enrolled 8/28/15) record did not include a second emergency contact and an employment address for one parent. Plan of Correction: The child's family was contacted and we were able to document on child's emergency card 2nd emergency contact and employment address fro one parent.
Standard #: 22VAC40-185-260-A Description: Based on documentation review, the center did not have documentation of a current annual fire inspection. Evidence: The last fire inspection was dated 11/12/14. Plan of Correction: The center contacted the fire official to request a fire inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not maintain areas and equipment on the playground in a safe and operable condition. Evidence: The wooden fencing along the left perimeter of the playground has rusty, protruding nails in every section, a split and splintered vertical wooden post in the first section, and a loose section of fencing. Plan of Correction: Director is working with District Manager and Facilities Tech on resolutions. A follow up plan will be communicated to Inspector Marty Jo Quade within the second week of January. Thursday Jan. 14, 2016 we will contact Ms. Quade.
Standard #: 22VAC40-185-440-L Description: Based on observation, the center did not ensure that pillows were not used by children under two years of age. Evidence: During the designated rest period, a 19 month old toddler was using a small pillow on her cot. Plan of Correction: Director spoke with all staff concerning safe sleep policy and regulation on the use of pillows for children under 2 yrs. A copy of the regulation was given to all staff. The child's pillow was sent home w/parent.
