Technical Assistance:
Discussed with the Director: Phase III Training on 2/2/16; Infant/Toddler specialist program.; Violation notice posted so that it is visible to parents; The changes in the Children of America biting policy is to be in writing no later than 2 weeks; Second component for late snack; moving the diaper disposal container closer to the diaper changing table; writing center for the school age classroom.

Comments:
An unannounced renewal inspection was conducted with the Director. The Inspection began at 9:15 and ended at 1:00. There were 46 children present with 9 staff and the Director and Assistant Director. Children were observed participating individual and small group activities. Activities were age and stage appropriate and classrooms were decorated with children's work. Reviewed children's files - see violation notice Reviewed staff files = see violation notice Emergency escape drills and shelter in place drills are up to date as are Health and Fire Inspection. Medication was reviewed - documentation is complete. Menu was reviewed - Breakfast today was cereal, peaches and milk. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of 5 staff files, it was determined that 2 staff did not have TB screening on file. Both staff had been employed for more than 21 days. Evidence: Staff A , employment date of 9/28/15 and Staff B, employment date of 8/21/15, have no had TB screening.

Plan of Correction: The Director has notified both staff and they will have their TB Screening on file within the next 5 days. In the future, all staff will have within 21 days.

Standard #: 22VAC40-185-60-A
Description: Based on a review of 8 children's files, it was determined that 1 child's file was not complete. Evidence: The file for Child #4 had no emergency contacts other than parents on file.

Plan of Correction: The Director called and left a message for the parent during the inspection. Emergency contacts will be on file for all children.

Standard #: 22VAC40-185-80-A
Description: Based on observation, it was determined that one classroom had an attendance form that was not up to date. Evidence: The attendance form for Infant A had only 1 of 8 children signed in. That child was signed in at 6:00 a.m.. The classroom was inspected at approximately 10:30 and there were 8 infants in care at that time.

Plan of Correction: The Director will counsel the staff in that classroom and will monitor daily.
