22VAC40-191 Background Checks (22VAC40-191)

Today an unannounced monitoring visit was conducted beginning at 10:45am. There were 13 children present today in the Explorers and Pathfinders classrooms with three teachers and the Director. Children were observed eating snack, having safari creative play, and having story time. There are currently no children enrolled that require medications on-site. Daily health observation, MAT (medication administration training), and current First Aid/CPR certifications are all current for staff. Violations observed on the playground at the initial study have been corrected. Fire drills and shelter-in-place drills are current and documented. This inspection, including the exit interview, ended at 12:30pm. Should you have any questions, please call.

Standard #: 22VAC40-185-60-A
Description: Based on record review, three children's records (#1,2 and 4) are missing complete required documentation in their record. Evidence: Records #1 and 2 are missing documentation of proof of identity. Record #4 is missing documentation of a second emergency contact with a complete address and phone number.

Plan of Correction: I will ask the parents to provide me with the documentation today. We will make sure all records include this documentation.

Standard #: 22VAC40-185-280-B
Description: Based on direct observation, cabinets with cleaning supplies do not have locks. Evidence: In the Pathfinders and the Explorers classroom, cleaning supplies are stored in a cabinet out of reach of children. However, the cabinets do not have a locking mechanism to prevent access by children.

Plan of Correction: We will place child-proof locks on the cabinets today.

Standard #: 22VAC40-185-280-G
Description: Based on direct observation, substitute containers with cleaning solution do no have a label indicating their contents. Evidence: Two bottles containing a bleach and water solution in the Pathfinders classroom did not have a label indicating it's contents.

Plan of Correction: The bottles were labeled during the inspection. We will make sure substitute containers are labeled.

Standard #: 22VAC40-185-340-D
Description: Based on record review and observation, two staff members (#4 and 6) who are not qualified as a program leader are present in a classroom with a group of children and no other staff. Evidence: Both staff #4 and #6 have listed qualifications, education and experience to be a program leader, however, there is no official documentation of their highest level of education in their record.

Plan of Correction: I will have both staff bring in their education documentation immediately and place it in their record.
