During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 2/13/2025, as a part of the licensure period. The on-site inspection began at 10:25am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 33. The inspector reviewed 5 children?s records and 3 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-100-A
Description: The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child #1, who had been in care for over 30 days.

Plan of Correction: Provider will request a copy of the most recent physical exam from the parent.

Standard #: 8VAC20-800-100-B
Description: Physical examinations prior to attendance shall have been conducted within two months prior to attendance for children six months of age or younger. The physical exam in the record for Child #4 was conducted 6 months prior to Child #4's first date of attendance. Child #4 was 6 months old at first date of attendance.

Plan of Correction: Provider will request documentation of a more recent physical for Child #4.

Standard #: 8VAC20-800-90-A
Description: Before attendance, documentation of immunizations is to be obtained for each child.
The documentation of immunizations for child #4 was obtained 8 days after the child's first day.

Plan of Correction: Provider will ensure immunizations are obtained prior to child's first day.

Standard #: 8VAC20-800-570-A
Description: The provider shall ensure that a caregiver does not exceed 16 points. The total number of points permissable for two caregivers present is 32 points. There were two caregivers present during the inspection with a total of 33 points.

Plan of Correction: Provider will look for a temporary assistant caregiver to ensure each caregiver is within required number of points, or will adjust daily attendance to remain within the required number of points per caregiver.
