Comments:
The licensing inspector conducted an unannounced monitoring inspection January 7, 2016 from approximately 10:00am to 1:20pm. The following ratios were observed with a total of 15 children present during the inspection: Toddlers & Twos - 8 children with 2 staff Threes & Fours - 7 children with 1 staff School Age children were not present during the inspection. During the inspection, the Toddlers and Twos were observed dancing to the ABC?s song, building with large waffle blocks, taking bathroom breaks, and eating lunch. The Threes & Fours classroom was observed playing musical instruments, creating drums from paper plated, dancing and playing dress up, building with connector blocks and eating lunch. Later children were observed transitioning into naptime. All children were observed resting on their cots with the required linens. Lunch consisted of peanut butter and jelly sandwiches, potatoes, applesauce, salad and milk. All areas of the center including classrooms, hallways, bathrooms, and playground were inspected. The center was equipped with toys and supplies and items were available to the children. The van used to transport children was checked along with the required transportation documents and supplies. The owner stated medication is no being administered at this time. The following information was reviewed: emergency supplies, emergency preparedness plan, injury prevention plan, injury reports, fire and shelter-in-place drills, daily attendance, parent agreements, and staff training information. During the inspection, four children records and three staff records were reviewed. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of three staff records and interview, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date 12/15/15) did not contain the following: 1. Documentation of two character references prior to beginning employment. One documented reference was in the record. 2. Documentation of orientation conducted before the first day of assuming job responsibilities. The owner stated orientation was conducted prior to Staff #2 assuming job responsibilities but the orientation was not documented.

Plan of Correction: I will have Staff #2 complete the orientation form.

Standard #: 22VAC40-185-260-B
Description: Based on interview and observation, the center did not ensure approval from the health department was obtained at least on an annual basis. Evidence: During the inspection, the licensing inspector asked the owner to see a copy of the most recent health inspection. The owner stated she sent the original health inspection to the food program. The owner called the licensing inspector on 01/09/16 and left a message stating most recent inspection was conducted in July 2014. The owner stated the local health inspector came out and conducted an inspection on 01/08/16.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the center did not ensure the most recent inspection of the facility was posted. Evidence: During the inspection, the licensing inspector observed the posted documents from the licensing inspection was dated 01/07/2014. The most recent licensing inspection was conducted on 12/01/15. The center is required to post the finding from the most recent inspection of the facility.
