An unannounced renewal inspection was conducted today. Observed 1 child directly supervised by 1 staff member. Ratio and supervision were in compliance. A sample of 1 child's record, and 1 staff record were reviewed. The physical plant, emergency supplies and procedures were also reviewed. No medications were reported to be on site. The child was engaged in various activities, to include: free play, puzzles, and hand washing. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-60-A Description: Based upon documentation reviewed, a child's record was incomplete. Evidence: Child A's file was missing the following information: name and phone number of child's physician, name, address, and phone number of two designated people to call in an emergency if parent cannot be reached and allergies and intolerance of food, medication, or any other substances, and actions to take in an emergency situation. Plan of Correction: Child's file will be completed by the end of the day (01/06/16).
Standard #: 22VAC40-185-240-D-5 Description: Based upon interview, there was not at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: Staff A did not have the DHO training and she was the only staff member on duty during today's inspection. Plan of Correction: DHO training will be completed by the end of the week.
Standard #: 22VAC40-185-290-3 Description: Based upon observations made, electrical outlets did not have protective covers. Evidence: Two electrical outlets were missing two of the two required protective covers in the children's classroom. Plan of Correction: Electrical outlets will be covered with protective covers by the end of the day.
Standard #: 22VAC40-185-550-B Description: Based upon documentation reviewed, the emergency preparedness plan was incomplete. Evidence: The emergency preparedness plan was missing the contact telephone number for a back-up officer. Plan of Correction: Em. Preparedness Plan will be completed by the end of the week.
