Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: There was a large screen television on a cart in the preschool classroom. It had a strap around it however it was secured to the wall by one screw that was visibly loose and coming out of the wall, posing a tipping hazard.

Plan of Correction: The director is aware of this and has this on her maintenance list for repairs.

Description: Based on observation and interview with the director the center failed to ensure there was at least one standard size toilet provided for the school-age children. Evidence: The toilet in the school age classroom had a sign taped to the toilet that read, "Out of Order." The director stated the school age children have been using the 2 year-old bathroom that contains 2 small toilets appropriate for the 2 year-old age group.

Plan of Correction: The school age children will be taken into the main building to use the standard toilet next to the kitchen.

Description: Based on a record review the center failed to implement monthly practive evacuation drills. Evidence: The document the center uses to record fire drills did not have a fire drill recorded since August 2015.

Plan of Correction: The director has had monthly fire drills however has not documented them. She will ensure all fire drills are documented each month.
