An unannounced monitoring inspection was conducted on 1/12/16 from 10:45am - 1:35pm. During the inspection there were 109 children ages 4 months to 12 years old in care with 23 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, eating lunch and playing outside. Records were reviewed for five children and five staff. Medication was reviewed along with emergency procedures and emergency supplies. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-130-B Description: Based on a review of five children's records, it was determined that the licensee did not ensure that that the center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #5 (date of birth 11/17/12) contained an immunization record dated 10/21/13. 2. Staff #6 (Program Director) confirmed updated immunizations records had not been received from child #5. Plan of Correction: Child #5?s immunization update was overlooked by the administration. The parent of child #5 was contacted during the inspection visit and an updated immunization was obtained the following morning. In the future, the director as well as the assistant director will audit files to ensure that required updates are not overlooked.
