Comments:
Unannounced inspection was conducted from 9:45 10 10:45 to investigate a complaint related to the Care, Protection and Guidance of Children in the older Infant classroom. Base on interviews with the staff, review of the accident reports and attendance sheets, the complaint was determined to be valid. A response to the complaint with actions to be taken has been developed with the Director. The actions will be monitored by the Licensing Inspector.

Description: Based on interviews with staff, review of accident reports, review of attendance sheets and review of the biting policy, it was determined that the staff failed to ensure the care protection and guidance of the children in the Infant B class. Evidence: On 1/13/16 at 9:20 a.m. Child #1 was bitten on the finger by Child #2. Child #2 is a known biter. There were 5 children in the classroom with Staff A and Staff B. On 1/14/16, Child #1, at 10:50 a.m., was under the cribs and when he exited the crib, he bumped his head on the yellow metal piece, resulting in a bump on his forehead and a scrape on it. There were 5 children in the room with Staff A and Staff B. On 1/15/16 , Child #1 was bitten by child #2 on the forehead. The incident occurred at 2:30 p.m. There were 5 children in care with Staff A and Staff C, Staff B was just in the process of returning to her classroom from her break.

Plan of Correction: 1. The Director will again discuss the biting policy with the Corporate staff so that the new policy can be disseminated to families. 2. Babies will no longer be allowed to play under the cribs. The Director will try to order a play tunnel . 3. The Director will set up new procedures for staff who are entering and exiting during break time including a transitional phrase to assure that all staff know who had supervion responsibities and to assure that children are being monitored at all times. 4. When there is only one staff in the classroom, they will call management to assist in supervision when they are diapering.
