The licensing inspector conducted an unannounced renewal inspection January 14, 2016 from approximately 2:30pm to 6:10pm. The licensing inspector observed a total of 90 children being supervised by 14 staff members during the inspection. During the inspection the children were observed participating in a variety of activities. Children were observed playing games, playing outside, working on I-Pads and playing video games, completing homework, playing board games, and playing with a doll house. Snack consisted of juice and mozzarella cheese sticks. Staff was observed helping children with homework and engaged in conversations with children while they participated in various activities. All areas of the facility were inspected including classrooms, restrooms and hallways located in Nunnally Hall, Luck Hall, and Reeves Hall. Outdoor areas were also inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: updated injury prevention plan, daily attendance, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The facility's first aid kit and emergency supplies were inspected. Medication is administered and medication and medication authorization forms were reviewed.
During the inspection, five children records and five staff records were reviewed. The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection, the licensing inspector observed three containers of disinfectant wipes sitting on a shelf above a hand sink located in the Kinder Quest (Pre-K / Kindergarten) classroom. The containers were labeled with the words "Warning: Keep out of the reach of children". Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-330-B Description: Based on observations and measurements taken, the center did not ensure an adequate depth of resilient surfacing was located in all fall zone areas. Evidence: During the inspection, the licensing inspector observed the following: 1. The slide located on the Pre-K / Kindergaten playground had approximately five inches of mulch at the end of the chute. The chute was located over dirt without resilient surfacing on either side of the chute. The top of the chute is approximately two feet off of the ground. If a slide is defined as an embankment slide, an approved resilient surfacing at least six inches in depth needs to extend at least four feet out at the end of the slide. In order to be meet the definition for an embankment slide the top of the slide cannot more than one foot off of the ground. If the slide is not lowered and remains over one foot in height at the top, then approved resilient surfacing at least six inches in depth on each side of the chute needs to extended at least six feet out on each side and six feet at the end of the chute. 2. Two swing bars located on the Pre-K / Kindergarten playground, one bar approximately 3 feet 8 inches tall and one bar approximately 4 feet tall was located over mulch. The mulch under the 3 feet 8 inches tall bar measured 2 inches in depth. The mulch under the 4 feet tall bar measured 4 inches in depth. The bars required an approved resilient surfacing at least six inches under the bars and at least six feet out around the swing bars. Plan of Correction: We will rope off the areas and not allow the children to use the equipment until a decision is made about a long-term solution.
