Comments:
An unannounced monitoring inspection was conducted on January 26, 2016 from approximately 12:20 P.M. until 5:15 P.M.. At the time of entrance there were 69 children and 9 staff members present with children. The sample size consisted of 5 children's records and 7 staff records. Five medications were reviewed. Children were observed during nap time and riding scooters and tricycles in the multipurpose room. Children were also observed eating snack. Previous violations were reviewed. The playground was not inspected due to inclement weather. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed and verified with the center director during the exit interview. Please contact the licensing inspector with any question at 404-0568.

Standard #: 22V40-191(BC)-50-A-1-a-1
Description: Based on a record review and staff interviews, the licensee did not ensure that all staff do not have a criminal conviction that is a barrier crime or is any felony conviction within the last five years. Evidence: During the inspection on January 26, 2016, the record for staff #5 included a criminal records check listing a barrier crime, (misdemeanor 18. 2-57) with a conviction date of 8-11-2006. This misdemeanor requires that an individual may only be hired if 10 years have elapsed following the conviction.

Plan of Correction: Staff #5 will no longer be working in the center as of today.

Standard #: 22VAC40-185-130-A
Description: Based on a record review and staff interviews, the licensee did not ensure each child attending the center had documentation in their record of receiving immunizations required by the State Board of Health before the child can attend the center. Evidence: During the inspection on January 26, 2016 the record for child #4 (first date of attendance 1-18-2016) did not include an immunization record. Child #4 was observed in the center during the inspection.

Plan of Correction: The center director will obtain the immunization record.

Standard #: 22VAC40-185-160-A
Description: Based on a record review and staff interviews, the licensee did not ensure each staff member submitted documentation of a negative tuberculosis (TB) screening. Documentation of the screening must be submitted no later than 21 days after employment or shall have been completed within 12 months prior to or 21 days after employment. Evidence: During the inspection on January 26, 2016, the record for staff #5 (date of hire 11-23-2015) did not include documentation of a TB screening.

Plan of Correction: Staff #5 will no longer be working in the center.

Standard #: 22VAC40-185-60-A
Description: Based on a record review and staff interviews, the licensee did not ensure each child had a complete record. Evidence: During the inspection on January 26, 2016 the following records were determined to be incomplete. 1) The record for child #1, child #3 and child #4 did not include the employment phone number for one of the parents listed.

Plan of Correction: The center director will have the parents update the records today.

Standard #: 22VAC40-185-70-A
Description: Based on a record review and staff interviews, the licensee did not ensure each staff record was complete. Evidence: During the inspection on January 26, 2016 the record for staff #4 did not include the written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position. Staff #4 is designated as a lead teacher/program leader.

Plan of Correction: The center director will obtain the transcripts.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on January 26, 2016, in room 108 there was 1 spray container of bleach and water sitting on top of the cabinet and not stored in a locked place.

Plan of Correction: The center director will retrain the staff today.

Standard #: 22VAC40-185-340-D
Description: Based on a record review and staff interviews, the licensee did not ensure that in each grouping of children there was at least one staff member who meets the qualifications of a program leader. Evidence: During the inspection on January 26, 2016, staff #4 was observed working in the infant room and the center director confirmed staff #4 is the designated infant lead teacher. There was not documentation available during the inspection indicating that staff #4 meets the qualifications of lead teacher/program leader. Center director confirmed that there was not a program lead qualified staff member working in the infant room during the inspection.

Plan of Correction: The center director will obtain the transcripts and diploma.

Standard #: 22VAC40-185-440-B
Description: Based on observation and staff interviews, the licensee did not ensure cribs, cots, rest mats, and beds were identified for use by a specific child. Evidence: During the inspection on January 26, 2016, in room 106 there were 2 children observed sleeping in cribs that were not labeled with their name or identified for use by a specific child.

Plan of Correction: The center director will retrain the staff and cribs will be labeled tomorrow.

Standard #: 22VAC40-185-450-D
Description: Based on observation, the licensee did not ensure that when pillows are used during rest time that they are assigned for individual use and covered with pillow cases. Evidence: During the inspection on January 26, 2016, during nap time 4 children in room 105 and 1 child in room 102 were observed sleeping on pillows without pillow cases.

Plan of Correction: The center director will retrain the staff and send a letter home to the parents.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interviews, the licensee did not ensure that when cloth diapers are used, a separate leakproof storage system is used. Evidence: During the inspection on January 26, 2016, 2 children in the infant room were observed with cloth diapers and there was not a separate leakproof storage system that is either foot operated or used in such a way that neither the staff members hand nor the soiled diaper touches the exterior surface.

Plan of Correction: The center director will purchase containers tomorrow.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and staff interviews, the licensee did not ensure the findings of the most recent inspection of the facility were posted for viewing. Evidence: During the inspection on January 26, 2016, the results of the most recent inspection dated August 31, 2015 were not posted.

Plan of Correction: The center director will ensure licensing results will be post today.
