Technical Assistance:
Medication was discussed with the program director: It is recommended that when accepting medication into the center that authorization form is clear and specific in regards to dosage and special instructions.

Comments:
An unannounced monitoring inspection was conducted from 9:40am-11:25am. Upon arrival there were 60 children in care with 15 staff present. A sample of 3 staff and 4 children's records were reviewed in addition to 6 medications and 1 over-the-counter skin product. The children were observed during "Stretch and Grow," playing outside, in centers and during circle time. A diaper change was also observed.

Violations:
Standard #: 22VAC40-185-290-3
Description: Based on observation the center failed to ensure electrical outlets had protective covers. Evidence: In the school age room a preschool classroom of 18 four year-old children were observed participating in a "Stretch and Grow" class. There was an uncapped electrical outlet on a power strip attached to the wall, next to the computer.

Plan of Correction: This was an oversight and the outlet was capped during the inspection.

Standard #: 22VAC40-185-430-B
Description: Based on observation the center failed to ensure materials and equipment were age and stage appropriate for children. Evidence: In the 2 year-old room there were uncooked pieces of large ziti macaroni in the sensory table. Due the size, shape and consistency of the macaroni, they posed a choking hazard to this age group. In addition the macaroni pieces fit inside of a choke tube.

Plan of Correction: The macaroni was removed from the sensory table during the inspection and will be replaced with items that are appropriate for 2 year-olds and do not pose a choking hazard.
