Technical Assistance:
Keep an eye on the grass growing in the mulch on the preschool playground. The grass in not in the fall zone of the climber however, if the grass spreads it may encroach into the fall zone. The area of peeling paint on the outside of the preschool exit door should be removed before it becomes a safety hazard.

Comments:
An unannounced monitoring inspection was conducted on 1/26/2016 from 9:45am - 11:55am. The following staff to child ratios were observed: *2's - 16 x 2 staff *3's - 11 x 2 staff *4's - 26 x 3 staff Children were observed during morning program time and lunch. Hand washing was reviewed in all classrooms. Positive behavior guidance was observed in the two-year old and four-year old classrooms. Additionally reviewed: medication, emergency supplies, transportation procedures, written emergency procedures, updated injury prevention plan, the posted menu, posted evacuation procedures, three staff records and five children's records.

Description: Based on record review, the center failed to ensure that the written record of children's minor injuries in which entries are made the day of occurrence shall contain staff and parent signatures or two staff signatures. Evidence: An injury report, dated 1/8/2016, lacked a parent or second staff signature.

Plan of Correction: The parent was notified of the injury on the day of occurrence. However, the child was not picked up by the parent on this day. The child in not usually picked up by the parent. A second staff signature was added during the inspection.
